On this page you will find the forms relating to the disciplinary process and Tribunals.

This form is the Request for a Tribunal Hearing-Appeal Hearing. This form should be used if you have received:

• a Disciplinary Infringement Notice for an act of Misconduct (normally this involves having been sent off during a Match) or
• if you have already attended a Tribunal Hearing but were not satisfied about the finding resulting from the hearing.

The last form is the (Request for a Grievance Hearing).This document is temporarily not available.  This form should be used if you wish the Tribunal to hear a Grievance.  For example, if your club has been sanctioned for a breach of the Rules of Competition, you should use this form.
